My son/daughter has permission to attend the Midwest Football Camp. I enclosed a $100.00 deposit,
with the balance due when he/she reports to camp. I understand that refunds will be subject
to a $50.00 cancellation fee. I, hereby, release the Midwest Football Camp, its staff and the
host institution from all claims due to injuries sustained by my son/daughter during the camp.
